Frequently Asked Questions About OCD and ERP Therapy
If you are struggling with OCD or anxiety, you probably have a lot of questions. This page covers the most common questions we hear from teens, adults, and families seeking OCD treatment at The OCD Therapists. We specialize in Exposure and Response Prevention (ERP), the gold standard treatment for OCD, and we serve clients in person in Tampa, Florida, and via telehealth in Florida, South Carolina, Utah, and New Jersey.
OCD is one of the most misunderstood mental health conditions in the world. Most people believe OCD is a problem with thoughts. It is not.
OCD is a cycle. A thought creates fear. Fear creates a compulsion. The compulsion creates temporary relief. And that relief teaches the brain to repeat the cycle again. Whether the compulsion is physical or mental, the result is the same.
The good news is that cycles can be broken, behaviors can be changed, and recovery is possible.
OCD can attach itself to almost any thought, fear, or theme. At The OCD Therapists, we work with teens and adults experiencing many forms of OCD, including:
Relationship OCD (ROCD) Harm OCD Contamination OCD Sexual Orientation OCD (SO OCD) Pedophilia OCD (POCD) Health OCD Religious OCD (Scrupulosity) Existential OCD Mental compulsions Other anxiety and obsessive compulsive spectrum disorders
While every person’s experience is unique, the underlying cycle of OCD is remarkably consistent across all themes. That is why ERP is effective regardless of which type of OCD someone is dealing with.
Many people believe compulsions are always visible actions like washing hands or checking locks. In reality, some of the most debilitating compulsions happen entirely inside the mind.
Common mental compulsions include:
- Rumination
- Mental checking
- Reassurance seeking
- Mental reviewing Analyzing
- Trying to figure things out
- Testing feelings
- Mental avoidance
Mental compulsions can occur within any OCD theme and are often the primary reason people remain stuck. Many people who believe they have Pure O are actually performing mental compulsions throughout the day without realizing it. Identifying and stopping mental compulsions is one of the most important steps in recovery.
Pure O is a term sometimes used to describe OCD in which a person does not appear to have visible compulsions. However, Pure O is largely a misconception. Most people who identify with Pure O are performing mental compulsions such as rumination, mental reviewing, reassurance seeking, and analyzing without recognizing them as compulsions at all. The obsessions may be internal, but so are the compulsions. ERP addresses both.
Yes. People recover from OCD every day.
Recovery does not mean you will never experience another intrusive thought, another moment of anxiety, or another spike of fear. Recovery means you no longer respond to those experiences with compulsions. When compulsions stop, OCD loses its fuel.
The skills learned through ERP stay with you for life. While fear is a normal part of being human, recovery teaches you how to respond to fear in a way that prevents OCD from controlling your life again. Our goal is not to help you become fearless. Our goal is to help you become free.
Exposure and Response Prevention, or ERP, is the gold standard treatment for OCD. It is the most researched and most effective approach available.
ERP works by gradually exposing a person to the thoughts, situations, or feelings that trigger fear, while helping them resist the urge to perform compulsions in response. Over time, the brain learns that fear does not require a compulsion to manage it. Anxiety naturally decreases, and OCD loses its grip.
ERP is not about eliminating fear. It is about changing your relationship with it.
Traditional talk therapy is often not designed for OCD. OCD wants you to analyze, process, solve, and find certainty. Unfortunately, many therapeutic approaches unintentionally reinforce those same patterns. The more someone tries to solve fear through conversation, the stronger fear often becomes.
ERP takes a different approach. Instead of trying to solve fear, we learn how to face it. Instead of chasing certainty, we learn how to tolerate uncertainty. Instead of performing compulsions, we learn how to stop feeding OCD.
The thoughts are not the problem. The fear is not the problem. The compulsions are the problem. That is why ERP remains the gold standard treatment for OCD.
Many clients at The OCD Therapists have tried therapy before coming to us. If previous therapy did not help, it is very likely that ERP was not part of the approach.
Many people spend years trying to control their thoughts. Others spend years trying to control their feelings. Neither approach works.
As human beings, we cannot always control what we think or what we feel. But we can control what we do. Recovery is not about controlling your mind. Recovery is about changing your relationship with fear.
The moment you stop obeying fear is the moment recovery begins. Because recovery does not begin when fear disappears. Recovery begins when we stop running from it.
Yes. The OCD Therapists provides ERP therapy for both teens and adults. OCD often emerges during adolescence, and early intervention with evidence based treatment can make a significant difference. Many clients choose to use out of network benefits through their insurance provider, which may allow for partial reimbursement. Online ERP therapy is just as effective as in person treatment and allows clients to work on recovery from the comfort of their own home.
The length of treatment varies depending on the individual, the severity of symptoms, and how consistently someone engages with ERP outside of sessions. Many clients begin to notice meaningful progress within a few months of consistent work. ERP is not a passive process. The skills are practiced between sessions, and the more fully someone engages with the work, the faster progress tends to come.
